† Engraftment is defined as achieving a peripheral blood absolute neutrophil count of 500 per microliter for three consecutive measurements with the first three measurements occurring within 100 days of transplantation.

Individuals whose residence is in Missouri, Oklahoma or Rhode Island. Individuals whose cord blood stem cell collection, storage or transplant fees are paid in full or part by Medicare, this includes third party payers in Massachusetts and Michigan.
Documentation:
To apply for the Quality Product Guarantee, a signed and notarized statement from the primary transplant physician confirming the failure of engraftment as described above will be required. Lifeline Cryogenics reserves the right to request laboratory reports.
